Diabetes, Hypertension & Cholesterol Care

Testing and management of diabetes, high blood pressure and dyslipidemia (cholesterol and triglycerides), with close follow-up.

  • Diagnosis and lab monitoring
  • Glucose, blood pressure and cholesterol control
  • Medication adjustment
  • Nutrition and lifestyle plan

Diabetes, high blood pressure and high cholesterol do not hurt, which is why many people in La Porte go years without knowing they have them or without controlling them. At Clínica Hispana Nueva Salud La Porte we make the diagnosis with our own lab, give you a plan you can actually follow and see you regularly, in Spanish or English and with no appointment.

Type 2 diabetes and prediabetes

We diagnose with fasting glucose and hemoglobin A1C, which shows your average blood sugar over the last 3 months. An A1C between 5.7 and 6.4 indicates prediabetes, and 6.5 or higher means diabetes. If you already have the diagnosis, we aim to keep A1C below 7 in most cases, with checks every 3 months until stable and then every 6. We adjust metformin or other medications based on your results, check your feet and kidneys once a year and help you read your glucose meter.

High blood pressure

A reading of 130/80 or higher on several occasions is already hypertension. We measure your pressure properly, seated and at rest, and if needed ask you to check it at home for a few days to confirm. Treatment combines medication, less salt and physical activity, and we check kidney function and electrolytes before adjusting doses. If you also have chest pain or palpitations, we do an electrocardiogram during the same visit.

Cholesterol and triglycerides

The lipid panel measures total cholesterol, LDL (the kind that clogs arteries), HDL (the protective kind) and triglycerides. Targets depend on your risk: someone with diabetes or a previous heart attack needs a lower LDL than a healthy person. When diet is not enough, we start statins and recheck liver and lipids 6 to 12 weeks later. High triglycerides respond strongly to cutting sugar, sodas and alcohol.

What follow-up includes

  • Blood work in the clinic: glucose, A1C, lipids, kidney and liver function, with results in 24 to 48 hours
  • Thyroid testing when weight or cholesterol cannot be explained otherwise
  • Prescriptions filled at our pharmacy, with low-cost options
  • A meal plan adapted to the food you actually eat at home, not a generic diet
  • A copy of your results so you can keep a record and compare at each visit

When to come even if you feel fine

If you are over 35, overweight, have relatives with diabetes or high blood pressure, or had diabetes during pregnancy, a yearly check makes sense. The complete general checkup for $99 includes a blood panel, A1C, urine test and medical consultation, and is the simplest way to know where you stand.
