Abscess Drainage

A red, hot, firm and increasingly painful lump, sometimes with a white spot in the center, is almost always an abscess: a pocket of pus the body cannot clear on its own. Antibiotics alone do not cure it; it has to be drained. At Clínica Hispana Nueva Salud La Porte we do it the same day you walk in, under local anesthesia, with no appointment and in Spanish or English.

Where they appear and why

Abscesses form when bacteria, almost always staph, enter through a hair follicle, a scrape, a bite or an area of friction. They are common in the armpits, groin, buttocks, thighs, back, neck and face, and also near the tailbone (pilonidal cyst) or along the bikini line after shaving. La Porte's heat and humidity, sweat under work clothes and diabetes all raise the risk. A small boil may resolve with warm compresses, but if it grows past half an inch, hurts a lot or has gone several days without opening, it needs drainage.

What the procedure is like

1. The physician examines the abscess, its size and whether there is fever or signs that the infection is spreading
2. The skin is cleaned and local anesthesia is applied around the area
3. A small incision is made and all the pus is drained; relief is immediate because the pressure drops
4. The cavity is rinsed and, if it is large, a gauze wick is left in so it keeps draining
5. It is covered with a dressing and we explain the aftercare

The whole thing takes 15 to 30 minutes. If the pus is sent for culture, in a few days we will know which bacterium it is and whether it needs a specific antibiotic. When there is surrounding cellulitis, fever, diabetes or the abscess is on the face, we prescribe an antibiotic, available at our pharmacy.

After drainage

You come back in 24 to 48 hours to remove the wick and check the cavity; sometimes one or two more wound-care visits are needed. At home, change the dressing once a day, shower after 24 hours and take a pain reliever if needed. The wound closes on its own from the inside in 1 to 2 weeks; it is not stitched, because closing it would trap the infection. Wash your hands before and after touching the area, do not share towels or razors, and wash clothing that touched it in hot water.

When it is urgent

High fever with chills, red streaks spreading from the lump, an abscess on the face near the eyes or nose, swelling that grows within hours, or if you have uncontrolled diabetes or a weakened immune system. If you also feel very ill or confused, go to the emergency room or call 911.

If they keep coming back

When abscesses return again and again, we check your blood sugar, evaluate whether you carry staph and set up a decolonization plan with specific soaps and ointments, plus recommendations for your skin and work clothing.
