Immigration Medical Exam I-693

If you are adjusting your status for permanent residence, USCIS will require Form I-693 completed by an authorized physician, known as a civil surgeon. At Clínica Hispana Nueva Salud La Porte we perform the complete immigration medical exam, with the tests and vaccines in the same place, and hand you the sealed envelope ready for your case, all explained in Spanish or English.

What the I-693 exam includes

• Medical history review and review of your vaccination record
• Complete physical exam: vital signs, vision, hearing, heart, lungs, abdomen, skin and general evaluation
• Tuberculosis blood test (IGRA), required from age 2; if positive, a chest X-ray is required, which we coordinate
• Syphilis blood test (ages 18 to 44) and gonorrhea urine test (ages 18 to 24)
• Mental health and substance use evaluation, part of the official questionnaire
• Required vaccines by age: influenza in season, Tdap, measles-mumps-rubella (MMR), varicella, hepatitis B, pneumococcal and any others you are missing; we give them here or record the ones you already have
• Form I-693 completed and signed, in a sealed envelope for USCIS, plus a copy for you

The whole process is usually completed in two visits: the first for the exam, samples and vaccines, and the second, 2 to 5 days later, to review the lab results and hand you the envelope.

What to bring

• A valid photo ID (passport, driver's license or consular ID)
• Your vaccination card or records, in any language; they save you vaccines and money
• If you had tuberculosis, a previous positive test or treatment, the documents and X-rays
• A list of current medications and diagnoses, with reports if you have them
• If your attorney or representative gave you specific instructions, bring them
• Glasses or hearing aids if you use them

No fasting needed. If you prefer, you can fill out the applicant section of the form with us at the clinic.

Validity and common mistakes

The I-693 is valid if signed within the time frames USCIS sets in relation to your application; we guide you so the signature date lines up with when you will file it. Do not open the sealed envelope: USCIS rejects it if opened. Check that your name and A-number match your application exactly. If you are missing vaccines, get them before signing; an incomplete form delays the case.

If you also need other documents

For parallel work or school requirements, we can do the physical exam or the drug test during the same visit. The tuberculosis skin test (PPD) is not accepted for the I-693, which requires the blood test.

Flat price, no insurance

Health insurance does not cover the immigration exam, so we work with a flat price that includes the exam, form completion and the required lab tests; any vaccines you are missing are priced separately, and we tell you the total before starting. We accept cash and cards.
